This is a fine post, but totally doesn’t address the issue of computerized systems. I have been working at a single HH organization for over three years, and I am more than competent with computer systems. I have identified numerous difficulties dealing with the computerization of charts in our case, the technique we use essentially transfers our old paper forms to a computerized version but doesn’t usefully assist us track orders, patient progress across visits, essential signs, etc. etc. And since our boss does not pay for wireless access (and the nurses did not use them when he did), essentially nobody checks the chart whilst with a patient. Well, I do, but it is so challenging to do it with our system and normally fruitless simply because so many notes have not been turned in, I never blame nurses for not checking.

Hence my inference is that the evaluation and information presented in this weblog are constrained by past traditional policy and academic wisdom and not insightful of our modern unfolding catastrophy.
